Tóm tắt Polysubstance use (PSU) is prevalent among people who inject heroin (PWIH) in Hai Phong, Vietnam, increasing health risks such as HIV, HCV, unsafe injecting practices, stigma, and mental health challenges. While cognitive behavioral therapy (CBT) has proven effective for substance use disorders (SUDs), its impact on PSU remains underexplored. This study evaluated an eight-session CBT intervention’s effectiveness in modifying substance use behaviors, reducing self-stigma, and improving self-esteem among PWIH with PSU. A randomized controlled trial assigned 100 participants to either a CBT group (n = 50) or a control group (n = 50). Key outcomes were assessed pre- and post-intervention. The CBT group showed significant reductions in substance use (Cohen’s d = − 0.93), self-stigma (Cohen’s d = − 3.23), and increased self-esteem (Cohen’s d = 3.37). CBT effectively reduces PSU behaviors and self-stigma while enhancing self-esteem, highlighting its potential as a critical treatment component.
